Disclosure Consent

I consent to Mello Insurance using my company name when mentioning Mello Insurance’s current partners. I also consent to Mello Insurance using my company name and logo on promotional materials including our website. This agreement can be modified or amended at any time by either party by giving 10 days email notice to the other party. This document will remain in effect until either party chooses to end it by notifying the other party via email.
